Chronic Care Management is a program created to assist Medicare patients, with two or more chronic health conditions, in coordinating care and to help prevent hospitalization.  At Simplicity Health, we have a team of medical assistants who are trained to reach out to patients on a monthly basis in order to check in, provide resources, assist in scheduling visits, and look into any additional needs a patient may have.

This is a program offered through Medicare and will be billed as such. Patients can typically expect to pay around $10 per month after meeting their Medicare Part B deductible.  Patients do have the ability to disenroll at any time.

These care coordinators will NEVER ask for sensitive personal information such as social security numbers, bank accounts, or insurance information.
