If this is your first time scheduling with Simplicity, your first visit will be a discussion of your medical history, medications, and treatments.  We call this an Establish Care visit.  Any other medical needs (physicals, vaccinations, etc) discussed will be in addition to the Establish Care information.  If you choose to be seen for more than the Establish Care visit or discuss anything outside of that visit, you will be billed a separate charge.

 

When you arrive at your first appointment, please have your insurance information and photo ID available at check-in.  You will be asked to fill out and/or sign a few forms allowing us to treat you.
